Aetna PO Box Lexington KY 40512: Which Box Is Which

There is no single Aetna PO Box Lexington KY 40512. Aetna, which is part of CVS Health, prints at least seven different boxes in that ZIP code, and each one does a different job. The box most members ask about is PO Box 14094, which Aetna’s current dental claim form names for dental claims. PO Box 14079 takes Aetna Voluntary Plans benefit requests and some provider appeals. PO Box 14088 is the Aetna Medicare mailing address. Before you mail anything, use the address printed on your own ID card, plan documents or claim form.

Last checked: September 23, 2026. Every box below was read on an Aetna-owned page or form on that date.

Why one insurer uses so many boxes in one ZIP code

A big insurer does not open every envelope in one pile. It splits mail by job. Dental claims go to one box. Medicare questions go to another. Provider appeals go to a third. Each box feeds a different team, so a letter in the right box skips a sorting step.

That is also why the same company name can sit on very different return addresses. If you want the wider picture of why insurers work this way, our guide on why health insurance mail comes from a PO box walks through it.

There is one small pattern worth knowing. On Aetna’s own forms, box 14079 is written as 40512-4079, and box 14094 as 40512-4094. The four digits after the dash repeat the end of the box number. So if the ZIP+4 on your envelope lines up with the box that way, it is at least consistent with how Aetna prints its own addresses. It does not prove who sent the letter, because anyone can copy an address.

14079 or 14094? Settling a common mix-up

This is the question most readers arrive with: “Which Aetna box do I mail my claim to?” Two of our older pages described these two boxes differently, so here is what Aetna’s own paperwork says today.

PO Box 14094 is the dental box. Aetna’s Dental Benefits claim form, which we read on September 23, 2026, is marked GC-8-13 (6-25). It tells both the employee and the dentist to send “the completed benefits request and the bills to: Aetna Dental, PO Box 14094, Lexington, KY 40512-4094.” Our page on who uses PO Box 14094 in Lexington covers that box on its own.

PO Box 14079 is a Voluntary Plans and provider box. The Aetna Voluntary Plans Benefits Request form, GC-7-41 (3-18), sends benefit requests for Med Premier, Fixed Benefits and Hospital Plan coverage to “Aetna Voluntary Plans, PO Box 14079, Lexington, KY 40512-4079.” Aetna’s provider disputes and appeals page also sends reconsideration requests from providers in 19 states to the Aetna Provider Resolution Team at the same box. An older Medical Benefits Request form dated 2-13, still hosted on aetna.com, uses the same box under the name Strategic Resource Company. For more on this box alone, see our page on PO Box 14079 Lexington KY 40512.

So the short version: dental claims go to 14094 on the current dental form. Voluntary Plans claims go to 14079 on their own form. Neither is a general “all Aetna claims” box.

Where CVS Health fits in

Aetna’s About Us page says: “Aetna is a proud CVS Health company.” That is why you may see both names in your mail, on your ID card, or on an Aetna web page.

We did not find any box in Lexington KY 40512 that CVS Health lists under its own name, separate from Aetna. Every box we could confirm in this ZIP code carries an Aetna name or an Aetna team. If a letter from 40512 carries a CVS Health brand you do not recognize, we could not confirm who uses that box. Call the number on your card or statement and ask.

Which box should you mail your claim to?

Do not pick a box from any list, this one included. Start with your own paperwork. Here is the order we suggest:

  1. The form you are sending. Aetna claim forms print their own “Mail to” address. The dental form and the Voluntary Plans form each name a box. Use the one on the form in your hand.
  2. Your member ID card. Check the back of the card for a claims address and a member services number. If the card shows an address, it wins over anything you read online.
  3. Your plan documents. Your benefits booklet or Evidence of Coverage explains where claims and appeals go for your plan.
  4. A phone call. If the card, form and booklet disagree, call the member services number on your ID card and ask which address to use.

Two cautions. First, in many cases the doctor or dentist files the claim for you. Check with the office before you mail a duplicate.

Second, do not mail money to a box unless your bill tells you to. The Contact Aetna Medicare Member Services page, which we read on September 23, 2026, lists “Aetna Inc., PO Box 14088” as the Aetna Medicare mailing address and PO Box 14066 for Member Services and D-SNP Customer Service. It presents neither as a payment address, and it says Aetna handles premium payments through InstaMed. Use the payment instructions on your own bill.

Boxes we could not confirm

Some Lexington 40512 boxes show up online next to the Aetna name. We could not confirm them on an Aetna page on the day we checked, so we do not list them as Aetna boxes.

  • PO Box 14546. An older page on this site named it as a dental box. We did not find it on any Aetna page or form we opened. The current dental form names 14094 instead.
  • Other Aetna Dental boxes. Aetna Dental’s professional contact page may list more dental addresses, such as appeals boxes. That page did not load for us on September 23, 2026, so we are not listing those boxes here.

If your envelope shows one of these boxes, or any 40512 box not in our table, we could not confirm who uses this box. That does not mean the letter is fake. It means you should check it through a phone number you already trust, such as the one on your ID card or a past statement.

Is a letter from a Lexington KY 40512 box really from Aetna?

A return address is not proof. Anyone can print “Aetna, PO Box 14088” on an envelope. What the address tells you is where a reply is meant to go. It does not tell you who mailed it.

Real insurance mail is usually easy to check against your own records:

  • It names you correctly and often shows your member ID or part of it.
  • It refers to a claim, visit, plan or date you can match to your own records.
  • It matches something you expect, such as an Explanation of Benefits after a doctor visit.

Be careful if a letter asks you to call a number printed only on that letter, pay by gift card or wire, or send your Social Security number back by mail. Look the number up yourself on your ID card instead. Our guide to a letter telling you to call a number explains how to check a number before you dial it.

If you get Aetna mail but have never had Aetna coverage, it may be a mistake, a mixed-up address, or a sign someone used your details. Call Aetna using a number from Aetna’s own website, not from the letter, and ask whether a policy exists in your name.

Why this page exists

Most results for this search are either Aetna pages that each show one or two boxes for one audience, or third-party pages that repeat addresses without saying where they came from. We put the confirmed boxes on one page, show where Aetna prints each one, and say plainly which boxes we could not confirm. PO box assignments change, so the “last checked” date at the top matters. If you are reading this long after that date, trust your own ID card first.
